Canyon Oaks Nursing & Rehabilitation focuses primarily on post-hospitalization transitional care. Our greatest success is the ability to help patients recover and discharge back to home or to a lower level of care! We employ our own therapists and operate a large, fully contained therapy unit. At Canyon Oaks Nursing & Rehabilitation Center in Canoga Park, CA, Seta came to rehabilitation after a serious hip fracture that required surgery and made walking difficult.

With determination and the support of our therapy team, she steadily regained her strength and mobility. In just a few weeks, Seta progressed to walking on her own and was able to return home safely.

We’re proud of Seta’s progress and grateful to our team for supporting her every step of the way. 💙
