Full-Time, On-call, RN, Case Manager (details below)

QUALIFICATIONS

1. Is a graduate of an accredited school of professional (registered) nursing. BSN preferred.

2. Is currently licensed as an RN through the Colorado State Board of Nursing.

SUMMARY OF JOB RESPONSIBILITIES

The RN provides skilled professional nursing care to the patient/family/caregiver specific to patient, as prescribed by the physician and in compliance with all regulations including, but not limited to, those established by the State Board of Nursing, and the policies of Fremont Regional Hospice.

RESPONSIBILITIES AND DUTIES

1. Provides quality patient care and ensures patient/family are treated with dignity and
respect.

2. Observes the patient for changes in condition and reports these changes and the patient’s/family’s/caregiver’s needs to the attending physician and Interdisciplinary Team (IDT) members. Record pain/symptom management and changes/outcomes as appropriate.
