!!! Attention med card patients !!!

The state is now requiring for you to provide a copy of your provider certification when you purchase from a dispensary. Listed below are ways to obtain.

A. I provided the certification to you at the time of your appointment. If you saved the paper, please take it to the dispensary with you.

B. If you did not save the paper, you can log into your state account. Once you are logged in, follow these steps:

1. Click on the "active" patient registration
2. Click on the provider certification tab
3. Click on the "active" certification
4. Print certification

C. You may contact me to reprint the certification, however, there will be an associated $20.00 fee.

As of 04/01/2024, prices for weight loss medications will increase. This is a direct reflection of the increase in price from the pharmacies.

Tirzepatide-0 to 50 units: $400.00 for 4 injections
