Diagnosing endometriosis typically involves two crucial factors: infertility issues and pain. Significantly, the correlation between pain severity and the extent of endometriosis is not directly proportional, as some women with extensive endometriosis may not experience pain, while others with pain can still achieve normal pregnancies. This underscores the complexity of endometriosis diagnosis. Various factors can cause pelvic pain, which may coexist with other diseases. A comprehensive approach is essential, and treatment should not exclusively focus on endometriosis. Surgical interventions require judicious consideration, with surgeons needing specialized expertise. However, even with optimal treatment, pain can recur. Recurring pain does not necessarily indicate endometriosis, often being attributed to coexisting conditions that were overlooked. Painful bladder syndrome, pudendal neuralgia, and pelvic muscle pain are frequently missed diagnoses. Additionally, patients with pain, with or without endometriosis, often have less grey matter and neurons in the midbrain, allowing pain signals to be enhanced rather than filtered.

I delivered a presentation on pelvic pain desensitization prior to endometriosis surgery at the American Global Congress of Endometriosis Specialists in Las Vegas. The event was highly successful, providing an excellent opportunity to network with colleagues and share my expertise in addressing Chronic Pelvic Pain, Interstitial Cystitis, and endometriosis. I extend my gratitude to Drs. Nezhart for the invitation. My efforts continue with an upcoming presentation at the Endometriosis Summit 032726-032926 in Orlando, for which I thank Sallie Sarrel for the invitation.
