Mission Statement of Purpose: To empower at-risk individuals to overcome the social determinants of health and racial inequities in order to elevate the health of Stark County. Vision Statement: To challenge systematic barriers and ignite change that ensures that everyone has access to the services, tools, and resources necessary to promote good health and wellbeing. Your Pathway to a Healthy Pregnancy and a Healthy Baby!

Our Purpose…

To reduce infant mortality by eliminating barriers to health and wellbeing for at risk pregnant women.

Our Mission…

To reduce fetal and infant deaths among at-risk minority women and families in Stark County by focusing efforts on improving access, through care coordination, to primary care and quality social services.

The Community Action Pathways HUB is a FREE program for pregnant women in Stark County. So if you are pregnant and you live in Stark County then you are eligible! We have Community Health Workers (CHWs) that assist pregnant women throughout their pregnancy and up until their baby’s first birthday.

Your Community Health Worker will support you, encourage you, empower you, and advocate for you. Your CHW will also help you make decisions while connecting you to services and resources in Stark County to help you on your Pathway to a healthy pregnancy and a healthy baby!

Your CHW Can Help Connect You To:

· Transportation

· Education

· Medical Insurance

· Baby Items

· And so much more!
