Located in the central Rocky Mountains just 12 miles from Glenwood Springs and 30 miles from Aspen, Colorado, Heritage Park Care Center, a Life Care Centers of America facility, is at the base of beautiful Mount Sopris in Carbondale, making our nursing home an ideal choice for individuals in need of short-term rehabilitation, long-term care or post-operative recovery. Focusing on inpatient and outpatient rehabilitation with 24-hour skilled nursing care, our in-house rehabilitation team works hard to give every patient and resident the best care possible. We learn the goals and preferences of our patients to provide a tailored care plan. Our 3,000 square-foot rehab gym features state-of-the-art equipment, such as an AlterG® Anti-Gravity Treadmill® and a therapy pool, helping individuals achieve their highest level of independence.

Today, National Caregivers Day, we recognize all types of caregivers 🫶: our associates who bring compassion and professionalism to their interactions with our residents; family members who lovingly care for their loved ones; and other professionals in the healthcare industry who take care of those who cannot take care of themselves.

Together, we bring more than just "services." We bring love, comfort, and the dignity every person deserves. ☺️
