I work primarily with children dealing with PANS and PANDAS, supporting families who are navigating sudden changes like OCD behaviors, anxiety, tics, and more.

These cases can feel overwhelming and confusing, especially when symptoms come on quickly and don’t seem to make sense.

My role is to help identify the triggers, calm the immune response, and walk alongside families with a clear plan so they’re not trying to figure it out alone.

What actually causes PANS and PANDAS?

At the simplest level, it starts with an immune trigger.

An infection

A toxin

A stressor

But instead of responding normally, the immune system becomes dysregulated and begins affecting the brain.

That is when symptoms show up:

OCD

Anxiety

Rage

Tics

Food restriction

We also see flares tied to things like back to school, illness, and even mold exposure.

This is why getting to the root matters.

We often get asked how to tell the difference between PANS, PANDAS, and autism.

The biggest clue is timing.

Autism presents early and progresses gradually.

PANS and PANDAS are abrupt.

A sudden shift in behavior, mood, or function is not something we ignore.

That distinction matters more than most people realize.

IVIG is often seen as the go to treatment for PANS and PANDAS.

But it is more nuanced than that.

It works by regulating the immune system, but it can also temporarily worsen symptoms and requires proper support.

In our practice, we use it in a small percentage of cases.

Because the goal is always to choose the right treatment, not just the most talked about one.

Mycoplasma is a common trigger we see in PANS.

It is a respiratory infection most people have had, but in some kids, it drives:

OCD
Anxiety
Rage
Food restriction

It can hide in the body and cause recurring flares if not treated properly.

This is one we pay close attention to.

One of Dr. Scott's patients just told him that he needs a nickname. She said "like Dr. Pimple Popper". So she suggested "Dr. Phobia Stopper". (We are hoping it sticks!)

Incidentally, this young patient came to our office with anxiety and a severe phobia of spiders. One hour later, after her R3 session, she walked with Dr. Scott to the basement of our office and reached behind the basement wall to feel the cobwebs there with complete calm.
