Personalized Lower Blepharoplasty with Fat Grafting

In eyelid surgery, there’s no “one-size-fits-all” approach. Every patient has unique needs, and I tailor my surgical techniques accordingly. For lower blepharoplasty, there are two common methods: the transconjunctival (inside the eyelid) approach and the transcutaneous (skin muscle flap) approach.

I frequently perform both and customize each based on the patient’s anatomy, combining them with fat transposition and/or fat grafting when needed. In this case, my wonderful patient benefited from the transconjunctival approach, meaning no external incision and a refreshed, natural result. This approach is ideal for patients with good skin quality who need contour adjustments without removing extra skin.

The true art of plastic surgery lies in making precise, subtle changes that collectively enhance natural beauty.

This case is one of my favorites because it demonstrates how a series of subtle changes can collectively create a beautifully rejuvenated look.

**Procedures Performed:**
1. **Upper Blepharoplasty** with brow fat pad reduction
2. **Neck and Jawline Sculpting** through submentoplasty and selective fat grafting
3. **Buccal Fat Reduction** for a subtly slimmer cheek area
4. **Fat Grafting Under the Eyes** to brighten the under-eye area

**Key Highlights:**
- **Upper Blepharoplasty:** This was tailored very differently from a traditional upper blepharoplasty. This patient had hereditary excess brow fat, and reducing this brow fat pad is a very specialized procedure that very few surgeons perform.
- **Neck and Jawline Sculpting:** Achieved through a small incision under the chin, enhancing the jawline and neck without skin removal. Notice how her mole has moved, indicating the depth of sculpting.
- **Buccal Fat Reduction:** Slimmed the cheeks conservatively for a natural look.
- **Fat Grafting Under the Eyes:** Brightened the under-eye area using yellow fat, enhancing overall facial harmony.

As you can see in the lateral view, the skin has beautifully redraped over the newly sculpted deep structures, giving a naturally contoured and youthful appearance. All this was done without any skin removal, showcasing the power of precise sculpting.
