Dear Patients,
It is with mixed emotions that I announce my retirement from active practice, effective December 2, 2019. It has been a great pleasure providing for your health care needs over the years, and it is not easy for me to give it up.
As of December 20, 2019, Dr. Shaheen Jacob will continue to take care of my patients. Dr. Jacob is a colleague of mine at Baylor Medical Center of Carrollton and at Texas Health Presbyterian Hospital of Plano since 2012. She is terrific choice for your continued care, a trusted colleague and I am glad to leave my patients in her capable hands.
Your medical records are confidential, and a copy can be transferred to another doctor or released to you or to another person if you wish.
If you decide to obtain a copy of records, please sign the release authorization form and return it to my office as soon as possible. Or you may call my office to make an arrangement to pick up the copy. There will be a $25 fee for one copy. This rate is set forth by the Texas Board of Medical Examiners.
After December 20, 2019, all of the records will be transferred to Dr Jacob. If you continue care with Dr Jacob, then there will not be a transfer fee for any records. She will be the legal custodian of all the records at the following address, and you may use this office number to schedule your future appointments:
