Carrollton Nursing and Rehabilitation Center Carrollton Nursing and Rehabilitation Center is a skilled nursing facility located in Western Georgia with a maximum capacity of 159 residents.

We pride ourselves on offering the finest of care by staffing the facility with the right people. Our staff retention rates speak volumes about the culture within our facility as Carrollton Nursing and Rehab features many employees with over ten years of experience with us. In fact, we have two employees with a combined 60 years of experience at this facility alone! Come meet some of the wonderful

people working hard every day to improve our residents’ lives! On each wing of the building, we staff an Activity Coordinator who specializes in tailoring activities to fit each individual’s needs and desires. We focus on small group activities and one-on-one interaction to ensure we deliver on our promise of exceeding resident expectations.
