The SFC program is a Medicaid Waiver program that pays a family member to take care of their loved ones that are elderly and/or disabled. Please feel free to contact our office to discuss available services and to send a referral for approval for the program.
SFC requirements to be eligible for financial compensation:
* You must live with the person receiving services.
* You are related biologically OR through marriage.
* The person you take care of is Georgia Medicaid-eligible and agrees to participate in the CCSP/SOURCE SFC programs. If needed, we can send referral to be approved for services of the program.

Position Overview: Proclaim Home care is seeking a full-time & part-time RN to join our team. The RN will deliver dedicated care to our home health clients. The ideal candidate must be punctual, reliable, and capable of managing care plans effectively. This is an exciting opportunity to make a difference in the lives of our patients and contribute to the growth and development of our company.
Responsibilities:
*Provide direct care to home health clients.
*Collaborate with other healthcare professionals, to develop and implement patient care plans.
*Care for disabled and geriatric patients in their home environment.
*Teach patients and caregivers self-care techniques.
*Check vital signs
*Review all medications each visit
*Complete documentation timely and accurately
*Supervisory visits of services
*Complete admissions of services (RN)
Qualifications:
*Current nursing license in the state of Georgia
*Current CPR certification
*Valid driver's license & reliable transportation
Employment Type:
*Full-Time & Part-Time
*Flexible schedule (can make your own weekly schedule)
