We are all getting older. We see it in the mirror and know that it is happening. But most of us do not necessarily “feel” older - we just look older. There are predictable signs of aging that happen to almost all of us. The skin starts to wrinkle and the structures of the face start to look more and more tired. Tummy tuck surgery is one of the highest patient satisfaction procedures that Dr. Stoeckel and Dr. Purzycki perform. It is a dramatic way to correct the changes that often occur after significant weight loss or pregnancy. The classic procedure accomplishes two major goals. The first is to tighten the abdominal wall muscles (re**us abdominis) and correct the separation of those muscles (re**us diastasis) that is often seen during and after pregnancy. The second is to remove the excess skin and fatty tissue from below the belly button to create a tighter and smoother contour to the abdomen.

Most of us have some excess fatty tissue that just won’t go away, despite our best efforts at diet and exercise. Liposuction has been and continues to be a very effective way to remove these trouble areas. But many of us would prefer to avoid the bruising, swelling, and downtime that is often associated with liposuction surgery.

What is CoolSculpting?
CoolSculpting offers another option for removing unwanted fat without some of the side effects associated with liposuction. It uses something called cryotherapy to literally freeze targeted fat cells. Fat cells are more sensitive than skin tissue cells to freezing temperatures and because of this, CoolSculpting can preferentially crystallize and destroy fat cells without causing damage to the overlying skin. The fat cells end up dying and then the body is able to eliminate them by natural processes.

Is CoolSculpting Right For Me?
The best candidates for the procedure are patients that have trouble areas that are resistant to diet and exercise. Patients with the best results are those that are already very close to their target weight because CoolSculpting is not an effective tool for weight loss.

What Should I Expect During The Procedure?
CoolSculpting treatments can be done in as little as 35 minutes. Patients are comfortably positioned in a chair or bed with an applicator placed on the area to be treated. When the treatment starts, there are several minutes of discomfort as the tissue reaches the optimal freezing temperature. But, once the goal temperature is achieved, the remainder of the treatment is very tolerable as patients are either watching TV, listening to music, reading, or doing work on their computers or phone. The treated area is then quite numb for a few weeks as the fat cells are going through the cell death process. Most patients have a sensation of mild itching during the recovery process. The results take several weeks to be realized and optimal results are seen at about 2-3 months. Many patients choose to have 2 sessions per treatment area separated by about 1 month for the best clinical results.

There is probably no other procedure that plastic surgeons perform that has seen more innovation in the last several years than liposuction. It is an ever-popular surgery that can very effectively improve body contouring when the right patient is selected. Liposuction is not a substitution for weight loss, but rather a tool to help patients that are having difficulty with “trouble spots” that seem to be resistant to healthy diet and exercise.

Our board certified plastic surgeons use the technology of Power Assisted Liposuction or PAL in his practice. PAL involves the use of a quickly oscillating or vibrating cannula to extract the excess fat. The difference between traditional liposuction and PAL is similar to the difference between carving a turkey with a kitchen knife and using an electric knife. There is less effort exerted by the surgeon and less tissue trauma delivered to the patient. This results in a smoother contour with slightly less swelling and bruising during the recovery process.

Arm lift surgery involves the removal of excess skin and fatty tissue from the upper arm. It is often the only way to effectively improve the contour of the arm after significant weight loss or aging. Many patients report that they want to get rid of their “bat wings” so that they can return to wearing short sleeve shirts or other form fitting clothing.

Why choose Wake Plastic Surgery?
Our surgeons use a technique utilizing Power Assisted Liposuction or PAL in all arm lift cases. The PAL is used to both remove unwanted fatty tissue and to separate the skin from the underlying tissues of the arm in a gentle fashion. The unwanted excess skin can then be removed with minimal swelling and bruising with minimal risk of injuring either nerves or lymphatics. This technique has led to achieving significantly better results with significantly lower side effects than when he used more traditional techniques in the past.
How bad is the scar?
The drawback to full arm lift surgery is the scar. There simply is no way to remove the unwanted skin without placing a scar on the arm that typically runs from the armpit to the elbow. Dr. Stoeckel and Dr. Purzycki prefer to locate the scar on the posterior aspect of the arm so that it is well concealed in most natural body postures. The scar certainly improves dramatically during the first year of recovery, but it is always perceptible and is something that needs to be understood before undergoing the procedure.

What can be done for the loose skin under the armpit?
In some cases, the excess skin of the arm is continuous with excess skin of the lateral chest wall. In these cases, extension of the scar onto the chest under the armpit is needed to fully improve the upper body contour. When this is done, it is considered an extended brachioplasty or flankplasty. The scar is longer, but the improvement in the laxity of the side of the chest is usually well worth it.

How bad is the recovery process?
Arm lift surgery does indeed have a difficult recovery process for most. The actual pain associated with the procedure is well tolerated, but there is a constant reminder of the procedure for several weeks due to the soreness associated with arm movements. A long sleeved compression garment needs to be worn for 1-2 weeks after surgery to minimize swelling.

Who is a good candidate for the surgery?
The best candidates are those who are relatively close to their ideal body weight. Patients that have loose skin rather than just large arms will have better results, even though liposuction will be performed on all patients. Good candidates have realistic expectations, especially in regards to the permanent scars associated with the procedure.
