Horizon Health and Wellness is a non-profit integrated health care organization that offers a wide range of health care services for all ages and stages of life. Horizon’s skilled physicians, clinicians, nurses, psychiatrists, and healthcare experts specialize in primary care, behavioral health and addiction treatment, while being dedicated to improving the health of the communities we serve. For more than 40 years Horizon Health and Wellness has been serving the health care needs of Arizona residents, providing access to an array of inpatient, outpatient, residential and habilitation services that addresses the whole person and promotes wellness. Patients can also access Horizon services from anywhere in the state of Arizona through the convenience of telemedicine. Horizon’s Recovery Village is a provider of adult substance use residential treatment programs. These unique facilities are located in a serene and secluded rural area of Casa Grande, AZ. It includes a:
- Residential Substance Use Treatment Facility for Women
- Residential Substance Use Treatment Facility for Men
- Residential Substance Use and Co-Occurring Treatment for Men and Women, and Supportive Housing. The residential treatment facilities provide participants with the care and comfort needed on the road to recovery. Services:
The 30-day program (unless otherwise specified) assists participants in identifying addictive problems, learning adaptive coping and social skills, changing lifestyles, accessing community resources, establishing support networks and accomplishing individual service/treatment goals. Programming is scheduled throughout the day and evening hours which include individual counseling, group education and therapy, aftercare, life skills training, job skills training, case management, and family groups as requested. Horizon’s multidisciplinary and experienced care team consists of clinicians, behavioral health techs, peer supports and support staff. The team works together with the participant to develop a customized treatment plan that is specific to their goals. Easy & Convenient Admissions:
A full spectrum of treatment is offered to adults over the age of 18 years of age with a dependence” or “severe” substance use disorder diagnosis. Horizon is contracted with ACH, Mercy Care, Health Choice, Banner, Magellan, and Care 1st. A stress-free admissions process is in place to efficiently connect participants with the care they need.
