I hate to say just how accurate these skits are.

PBM reform on a state and federal level is necessary for pharmacies to remain open

We take losses on a huge number of the prescriptions we fill, especially brand names. And though we have tried to cut some of our losses, there are certain patients who are homebound or otherwise in need and can’t go to another pharmacy, mail order, or specialty pharmacy, so we end up taking the loss anyway. There are days that our net profit is negative due to filling prescriptions that our patients need. No business can survive on negative reimbursements and paying someone (PBMs) for the “privilege” of doing business in their network, especially not pharmacies who work tirelessly every day for the good of the patient

Spartan, we couldn’t have said it better ourselves.

We’re dealing with the exact same issues as outlined in Spartan Pharmacy’s post. PBMs are KILLING pharmacies, especially the independents.

Please know that it’s not an easy choice to let a patient know that we are unable to fill a medication. One of the worst parts is that per pharmacy contracts we are not allowed to tell a patient when our reimbursements are underwater, but almost all brand name medications and even some generics are.

Please help us keep up the fight by calling your representatives in the government and calling out PBMs on their abhorrent practices

Alabama is doing right by its community pharmacies.
Pennsylvania needs to catch up, or more pharmacies will fall victim to the predatory practices of big PBMs. We’ve been asking our patients for a few years now to continue supporting proposals aimed at protecting independent and community pharmacies.
We are still fighting to stay open for our patients.

Please continue to call your senators and representatives, anyone you can, in support of PBM and pharmaceutical reform for the benefit of beneficiaries and pharmacies.

The fight against PBMs is far from over and it appears that Congress has taken a step back. Despite a near unanimous, bipartisan support, the vote on PBM reform has been canceled.

Pharmacy Benefit Managers (PBMs) are destroying independent and small chain pharmacies.
As of March 20, 2025, at least 326 pharmacies have closed their doors since 12/19/24, with 237 of those being independent. Those closures have been linked to the predatory and exploitative practices of the PBMs.
