Join our team at the Madison County Rural Health Council and make a meaningful impact in your community. As a Social Care Navigator, you’ll help connect individuals to essential resources like health care, food, housing, and transportation—while building strong relationships with local partners and supporting community outreach efforts.

We’re looking for someone who is compassionate, organized, and a strong communicator, with a passion for helping others and knowledge of community resources. Experience in public health, case management, or a related field is a plus.
