UnityPoint Health-St. Luke's Transitional Care Center St. Luke’s has helped patients successfully transition from hospital to home for generations. Our new St.

Luke’s Transitional Care Center is helping patients recover from illness or surgery and go home faster. Sometimes after an injury, surgery, or illness you no longer need to remain in the hospital but you are not confident or strong enough to return to your home. We are your bridge between a hospital stay and going home so that when you do go home you are successful and have a reduced chance of rehospitalization. Our caregivers and staff are committed to delivering the care you need when you need it. Our highly-skilled team of providers, nurses, social workers, and therapists design a personalized care plan for you with input and communication from the hospital doctors and your primary doctor. From daily visits by a nurse practitioner to a custom rehabilitation program geared at strengthening the body, all services are uniquely tailored to help you meet your individual health goals. Following a qualifying hospital stay, our services may be covered by Medicare. Private pay is also accepted for individuals wanting skilled nursing services for a short time prior to returning home.

We had such a fun St. Patty’s Day with our residents and staff! 🍀💚
The day was full of shenanigans—from Lucky Charms arts and crafts to a spirited contest to see who could dress up as the best leprechaun! We even went on a hunt for gold coins, which brought lots of smiles and laughter. 🌈✨
We wrapped up the celebration with a festive St. Patty’s Day party that everyone enjoyed!! We hope everyone had a good time celebrating with us!

This morning everyone enjoyed getting creative with rock painting, and the results turned out beautiful. This afternoon we kept the fun going by decorating shamrock cookies and, of course, enjoying eating them too! 🍪💚
