Every surgical plan should be unique and customized to each and every patient based on a variety of factors. By having an armamentarium of surgical approaches and techniques, both minimally invasive and open traditional, one can adapt for pathology, anatomic considerations, medical comorbidities, surgical risk, and previous surgery.

Patient is an 81 yo M with left L2 and L3 radiculopathy +\- L1 involvement. Physiologically active as he walks 4 miles / day and does 100 pushups per day despite a triple CABG in 06/2021. History of L1 and L2 laminectomy 3+ years ago. Underwent L L2-L3 and L3-L4 TF-ESI with notable improvement in neurologic symptoms - 8/10 -> 3/10.

Performed L1-L4 prone [PTP] lumbar interbody fusion with L1-L4 percutaneous screw fixation. Discharged within 24 hours with complete oxygen relief of neurologic symptoms and no back discomfort. EBL - 50 mL. Total lateral retractor time - 79 min. Total surgical time - 205 min.

While minimally-invasive procedures are often tempting, slick, and equally effective compared to open procedures, the art of medicine is understanding when and where to utilize each appropriately. These two cases were performed the same day, both done on an outpatient basis. First case - BMI 38, surgical time 3:30. Second case - BMI 28, surgical time 2:30. Both procedures were left-sided open L4-L5 TLIFs, utilizing both indirect and complete direct decompression. Both woke up with complete and immediate resolution of all preoperative leg and back pain. Tough to argue with excellent clinical and radiographic results…
