Surprise Valley Health Care District We are a Critical Access Hospital, Skilled Nursing Facility and Rural Health Clinic We are located in Cedarville, California in Modoc County.

Surprise Valley Health Care District consists of the Surprise Valley Community Hospital and the Surprise Valley Clinic. We serve four towns and two Native American groups in Surprise Valley - Ft. Bidwell, The Warner Mountain Indians, Lake City, Cedarville, the Cedarville Rancheria, and Eagleville. We also see patients from all over the county who choose to make the trip over Cedar Pass because of

our knowledgeable and caring physicians and staff. Our hospital has 26 beds - 22 skilled nursing beds, 1 acute beds, and 3 swing beds. We also have an emergency room with a physician on stand-by 24 hours a day. If necessary, critical patients are first stabilized and then flown out of our local airport to larger facilities in Redding,CA, Reno,NV or Klamath Falls,OR

We also maintain a volunteer ambulance service. Several of our staff have trained and become nationally registered EMT's. They volunteer their time to supply the citizens of Surprise Valley with 24 hour ambulance service. Lab and X-ray services are available from 8 am to 5 pm, Monday through Friday for outpatient services. For emergencies, there is staff on call to cover these services 24 hours a day. The Surprise Valley Clinic has several providers on staff. If you have a sudden need to see a doctor, we can usually schedule you to see one of our providers the same day. In addition, a podiatrist, a general surgeon, and an audiologist are available by appointment only. NOTICE OF VACANCY

Surprise Valley Health Care District is accepting letters of intent to be considered to fill a vacancy on the Board of Directors created by the resignation of Randy Hall effective March 25, 2026. Mr. Hall’s term in office expires December 4, 2026.
Notwithstanding any other provisions of the law, any registered voter residing in the Surprise Valley Health Care District may serve as a Board Member. Letters should contain the candidate’s physical and mailing address, occupation, and any other information for the Board of Directors’ consideration.
Letters of intent will be accepted up to close of business. Surprise Valley Health Care District must publish or otherwise post an announcement of a board vacancy no less than 15 days prior to the appointment of a successor.
This vacancy will appear on the regular meeting agenda for consideration and appointment of a successor by the Board of Directors.

Marlin Shaefer would like to share the following SVHCD & Ambulance Statistics:
116 ER visits Year to Date. Compared to 81 over the same period last year. (+43%)
Of those 18 were from outside the valley.
48 Ambulance runs Year to Date compared to 23 YTD in 2025. (+100% over last year)
16/48 Ambulance runs were transfers. Either Life Flights or ground IFT's by Ambulance.

From the CEO at Surprise Valley Health Care District, Frances Hannah:

I would like to apologize for the miscommunication that has emerged from the recent Rurallite article saying we are emerging from bankruptcy. In no way did I mean to imply that we were already to that point, nor did I intend to mislead anyone to believe that we were. We are currently still in bankruptcy and working diligently to get out of bankruptcy.
Secondly, the skilled nursing facility is currently down to three stars due to reporting that has not been done for payroll to CMS, known as PBJ Reporting. Over the past 6-months we have had staff turnover, as well as a required payroll software conversion preventing us from being able to submit this reporting in the allotted timeframe to CMS. We are working on getting the reporting submitted and hope to be back to five stars soon.
Again, I offer this public apology for any miscommunication.
