🌟 CHP Home Care & Hospice is honored to receive a $5,000 mini grant from Tri County ADAMHS in support of our annual Rays of Hope Grief Camp in Van Wert.

Grief Camp each summer provides a safe space for local youth to navigate their emotions and find healthy ways to cope with sadness and grief after experiencing loss. Thanks to this generous support, we can continue offering hope and healing to the young people in our community and make a lasting impact in their lives. We are truly grateful to Tri County ADAMHS and their support.

🏎He has 18 Cup wins including the Daytona 500 in ’08 and the Brickyard in 2013.

🏁In 2023 he was recognized as one of the top 75 drivers in NASCAR history!

🏎 He grew up in South Bend, IN and is actively racing in the SMART Modified Tour WINNING this past weekend in the Flying VA Classic at Dominion Raceway!

🏁He began his love of racing at just 4 years old, he grew up racing in this area and he’s coming back for Chamber Race Night – January 17, 2026

“Adult Day Centers can be beneficial in a variety of ways such as improving the recipient’s overall behavior and provide the caregiver with much needed time off,” said Jenni Whitaker of CHP’s Celina Adult Day Center. “We focus primarily on social interaction, activities, and Alzheimer’s and dementia care.”

⭐️ Social activities—interaction with other participants in planned, appropriate activities
⭐️ Transportation—door-to-door service
⭐️ Meals and snacks—participants are provided with meals and snacks, including those with special dietary needs
⭐️ Personal care—help with toileting, grooming, eating and other personal activities of daily living
⭐️ Therapeutic activities—exercise and mental interaction for all participants

Thanks to our Sponsor a Need Campaign, the Celina Eagles and VFW Celina Post 5713, we were able to put their donations toward this much needed visibility that lets the community know where our day center is located. It’s also helpful for our clients and caregivers too! 🙂

🐄 Today was so much fun! 🤩

Our Adult Day Center clients and staff enjoyed a tour of MVP Dairy! The tour was generously donated to our ADC Sponsor a Need Campaign 🙂 We had the best time learning about how their impressive facility operates! We are truly grateful for this experience today and can’t thank MVP Dairy enough for their knowledge and kindness! 🐮😊

Our group finished the tour with fresh yogurt 🥛

About Us

Community Health Professionals (CHP) Home Care & Hospice of Celina is a non-profit home health & hospice organization serving Mercer County and surrounding area.

What makes CHP unique is that we provide a complete range of services - including visiting nurses, in-home therapy, hospice, inpatient hospice, private duty home care and adult day services.

Our nurses and staff who are local and available 24-hours a day, seven days a week. Making a referral to CHP is easy...just call us! Our staff will help you decide what service is right for you and we will work with your physician. We are happy to provide a no obligation, informational visit by one of our friendly and caring nurses. She will be happy to answer any questions you may have.
