Our center gives caregivers a break while providing older and disabled adults with a safe, supportive place during the day. We offer personal care, medication help, meals, activities, and more to help clients stay at home and out of long-term care.

🍽️ At our Adult Day Center, we provide a nutritious meal to our clients every day. We support the local Fountain Restaurant who supplies our meals daily.

Thanks to a grant from Midwest Electric's Community Fund, we’re able to help offset meal expenses and keep our daily rates affordable for the clients, families, and caregivers we serve. 💚

We’re so grateful for this support—it helps us continue offering care that’s both compassionate and accessible to Mercer County and Auglaize County residents. Our Adult Day Center helps keep your loved one at home, before long term care placement. We also provide transportation to and from our center in Celina.

As a daily transporter of clients to and from the center, Mike sees first hand the crucial role CHP's center serves in the community and for caregivers.

About Us

Community Health Professionals (CHP) Home Care & Hospice of Celina is a non-profit home health & hospice organization serving Mercer County and surrounding area.

What makes CHP unique is that we provide a complete range of services - including visiting nurses, in-home therapy, hospice, inpatient hospice, private duty home care and adult day services.

Our nurses and staff who are local and available 24-hours a day, seven days a week. Making a referral to CHP is easy...just call us! Our staff will help you decide what service is right for you and we will work with your physician. We are happy to provide a no obligation, informational visit by one of our friendly and caring nurses. She will be happy to answer any questions you may have.
