A heartfelt thank you to the Mercer County Civic Foundation! A $6,000 grant was awarded to our Adult Day Center in Celina last week, this grant helps keep pricing affordable for your loved one at our center. Our center is a much needed community resource as we provide transportation to and from our center, a daily hot meal, snacks, activities, personal care, and medication management.

What a frightfully fun Halloween party today at our Celina Adult Day Center! 🧡 Clients, staff, and volunteers joined in for pumpkin painting, mask making, cookie decorating, ring toss, and a twist on the classic bobbing for apples. So many laughs and spooky smiles! 🎃 We hope everyone has a safe and fun Halloween!

🌟 CHP Home Care & Hospice is honored to receive a $5,000 mini grant from Tri County ADAMHS in support of our annual Rays of Hope Grief Camp in Van Wert.
Grief Camp each summer provides a safe space for local youth to navigate their emotions and find healthy ways to cope with sadness and grief after experiencing loss. Thanks to this generous support, we can continue offering hope and healing to the young people in our community and make a lasting impact in their lives. We are truly grateful to Tri County ADAMHS and their support.

🏎 He grew up in South Bend, IN and is actively racing in the SMART Modified Tour WINNING this past weekend in the Flying VA Classic at Dominion Raceway!

What makes CHP unique is that we provide a complete range of services - including visiting nurses, in-home therapy, hospice, inpatient hospice, private duty home care and adult day services.
Our nurses and staff who are local and available 24-hours a day, seven days a week. Making a referral to CHP is easy...just call us! Our staff will help you decide what service is right for you and we will work with your physician. We are happy to provide a no obligation, informational visit by one of our friendly and caring nurses. She will be happy to answer any questions you may have.
