Altius Home Health is a Colorado based home health agency, seeking a qualified Registered Nurse in our Denver Metro Area, preferably with home health experience, who is responsible and reliable, is able to work independently, has good communication skills and is dedicated to providing quality patient care at home. The perfect candidate will be responsible for the following:

1. Evaluates patients referred for home care to determine appropriateness for admission.

2. Initiates Plan-of-Care and makes necessary changes as necessary

3. Completes documentation accurately, and submits to agency office in a timely manner.

4. Establishes and promotes an ongoing communication with Agency personnel.

5. Travel to patient's home and provide nursing care if needed or assign LPN to the patient.

Qualifications:

Current and active license to practice as an RN in Colorado or multi-state license
Copy of updated TB results/Chest x-ray
Up-to-date first aid and CPR certification

Schedule:

Flexible, based on your availability (morning, evenings and weekends)

Job Types: Full-time, Part-time, Travel nursing, PRN

Pay: $50.00 - $105.00 per visit
