Stony Brook MIS division chief Dr. Konstantinos Spaniolas talking about the role of GLP1 agonists in management of obesity. Weight loss medications have a role in patients with BMI in high 20s-low 30s or as a “neoadjuvant” therapy prior to bariatric surgery in patients with extreme BMIs. GLP1 agonists provide 10-15% total body weight loss, while bariatric surgery - in 30% range. Both medications and surgery might be needed to achieve desirable weight loss in some patients.

Stony Brook Hernia team recently performed the fist robotic retromuscular parastomal hernia repair at Southampton Hospital. Robotic approach enables faster recovery and less complications. Mesh reinforcing the repair is isolated from abdominal cavity so the patient won’t get severe adhesions after the repair. This will make subsequent surgeries easier.

More mesh removals. This time not for infection, but for chronic pain. Inguinal mesh plugs are evil, as are any 3D (not flat) constructs. More scarring, more pain. They form severely scarred and contracted “meshomas” which are difficult to remove.

Femoral hernias are 10 times more common in women, than men. They account for 17-30% of all groin hernias in women, as opposed to less than 1% of male groin hernias. This is an example of unusual femoral hernia in a male, containing incarcerated omentum (intraabdoninal fat). Robotic preperitoneal repairs are the best for these hernias. They allow us to cover and reinforce all weak areas in the groin. Surgery is ambulatory and most patients return to work within a week.

If it looks and smells like chronic mesh infection, it must be it. Acceptance is tough, so we every now and then see patients who underwent index repairs at other institutions.
Some mesh infections can be salvageable. Some are not.
There are any risk factors of surgical infection after hernia repairs: smoking, poorly controlled diabetes, immunosuppression, contamination during surgery…
