The Access Center for Independent Living (ACIL) is doing a survey about unmet needs in the disability community. We would like to invite individuals, and organizations to participate in this survey.

This survey will ask basic questions about demographic data, disability status, and COVID-19 pandemic challenges and barriers. The information collected will help improve emergency preparedness and response related to future disasters, such as pandemics. Results can be used to improve efforts to have more accessible healthcare within the community.

There are two ways to take the survey: using the online link listed below, or by requesting a paper copy through
bergman.103@wright.edu. If an individual requests to complete a paper survey one can be mailed to them. An addressed return envelope and stamp will be included to submit the completed survey. This survey will be anonymous, and participants can voluntarily complete the survey. Individuals can complete the survey at their own discretion and with additional help from their caretakes or other support people if needed.
