To Former Patients and Their Families,
Dr. Stephen Grcevich ended his active clinical practice of child and adolescent psychiatry through the Family Center by the Falls on June 20th, 2025. Our office in the Winbury Professional Center closed on June 30, 2021.
The following information is provided to former patients of the practice or medical or mental health professionals requesting patient records.
For patients who received care from 2016 on, your medical records are in electronic form, organized in .pdf files. They are predominantly in the form of handwritten notes. All medical records are confidential and protected by federal privacy and security regulations. With your authorization, a copy of your/your child’s medical record can be released to you or transferred to a physician or licensed clinician of your choice. For patients who received care prior to 2016, nearly all of our records are paper. We retain patient records for ten years, or until patients reach the age of 28, whichever comes last, at which point the files are destroyed.
There is no charge for records transmitted electronically. If you require paper copies of records, a charge applies at rates established by the State Medical Board of Ohio. Be aware that progress notes from psychotherapy sessions with psychologists, counselors and social workers employed by the practice prior to June 2021 are not included in the records made available, because a different set of privacy laws apply.
For patients of Dr. Grcevich who require a typewritten summary of your/your child’s treatment, such a summary at your request. The charge for this service is billed at Dr. Grcevich's final hourly rate ($375), prorated for the time required to complete the written report.
The office phone number will be disconnected on or around June 30th, 2025. Dr. Grcevich plans to maintain his office e-mail (drgrcevich@fcbtf.com) for former patients and their families who need to get in touch with him. If you need medical records at a later date, the easiest way to ensure the records end up in the proper hands is to sign a release at the office of the clinician desiring the records and have them send the release to the fax line Dr. Grcevich plans to maintain in retirement - (440) 754-3904.
