Crouch Family Medicine Crouch Family Medicine ~ We care for you & your entire family. Crouch Family Medicine is the private practice of Jason Crouch, D.O.

It is privately owned and operated by Dr. Crouch. The practice opened in Chandler, Oklahoma on July 30, 2007. Unlike many medical practices in the U.S., Crouch Family Medicine is not owned or managed by any hospital or hospital group. This gives Dr. Crouch the freedom to refer you the specialist of your choice. There are many good hospitals and specialty clinics in the area and Dr. Crouch is happy to refer you to the hospital and specialist of your choice for your specific medical need. In April, 2012, we added another provider to the practice. JaNena Wilson, DNP, APRN, FNP-C is a Nurse Practitioner certified in Family Practice. JaNena received her BSN from OU School of Nursing, her MSN with emphasis in Family Nurse Practitioner from University of Missouri-Kansas City, and her DNP from University of Missouri-Kansas City. Before becoming a Certified Nurse Practitioner, JaNena spent 4 years as a RN in the Critical Care Unit at Deaconess Hospital in Oklahoma City. JaNena is a member of and Board Certified by the American Academy of Nurse Practitioners (AANP). She is also a member of the Oklahoma Nurse Practitioners (ONP). JaNena provides patient appointments on Mondays, Tuesdays, and Fridays and is currently accepting new patients of all ages. The clinic is a non-emergency facility offering lab services, Digital X-Ray, and EKG. Dr. Crouch specializes in and is Board Certified in Family Medicine.
