It is privately owned and operated by Dr. Crouch. The practice opened in Chandler, Oklahoma on July 30, 2007. Unlike many medical practices in the U.S., Crouch Family Medicine is not owned or managed by any hospital or hospital group. This gives Dr. Crouch the freedom to refer you the specialist of your choice. There are many good hospitals and specialty clinics in the area and Dr. Crouch is happy to refer you to the hospital and specialist of your choice for your specific medical need. In April, 2012, we added another provider to the practice. JaNena Wilson, DNP, APRN, FNP-C is a Nurse Practitioner certified in Family Practice. JaNena received her BSN from OU School of Nursing, her MSN with emphasis in Family Nurse Practitioner from University of Missouri-Kansas City, and her DNP from University of Missouri-Kansas City. Before becoming a Certified Nurse Practitioner, JaNena spent 4 years as a RN in the Critical Care Unit at Deaconess Hospital in Oklahoma City. JaNena is a member of and Board Certified by the American Academy of Nurse Practitioners (AANP). She is also a member of the Oklahoma Nurse Practitioners (ONP). JaNena provides patient appointments on Mondays, Tuesdays, and Fridays and is currently accepting new patients of all ages. The clinic is a non-emergency facility offering lab services, Digital X-Ray, and EKG. Dr. Crouch specializes in and is Board Certified in Family Medicine. Applications are now available for the 2025 Chandler Christmas Festival Queen Scholarship sponsored by Crouch Family Medicine!

All Chandler High School females in the sophomore, junior, or senior classes are eligible to apply for this $1,000 scholarship. The recipient will appear in the Chandler Christmas Festival and Parade on Saturday, December 6th. She will be awarded the scholarship at the conclusion of her festival duties.

The application can be obtained through Mrs. Cummings' high school counseling office and will be distributed by school email, through hard copy in the English classes, and on the school website.

The scholarship deadline is Monday, November 3rd. All applications should be returned by mail and postmarked on/by this date. The recipient will be contacted by phone in mid-November. The announcement of the winner will also be made on the Crouch Family Medicine Instagram page.
