Heritage Health Care Excels in Q2 2025 Clinical Results

Heritage Health Care is proud to share outstanding results from our Q2 2025 Clinical Scorecard. Our team earned a five-star rating in CMS short-stay quality measures—well above the national average.
In fact, 80.6% of our short-stay residents achieved or exceeded the expected ability to care for themselves and move around at discharge, compared to the national average of 54.9%.

More than half of our residents (54.2%) returned to their homes or communities, while only 20.8% required further skilled nursing. Our expanded therapy gym and dedicated inpatient/outpatient services continue to help residents regain independence and confidence.

With 24/7 RN coverage, 18 new private rooms with showers, and a focus on person-centered care, Heritage Health Care remains committed to excellence and recovery success for every resident.
