It is time for our February case of the month! This is a case of a 75 year old female with a history of chronic kidney disease, systemic lupus erythematosus, rheumatoid arthritis, and heart failure who was admitted to the hospital after an episode of acute chest pressure. The workup included a cardiac PYP nuclear imaging scan to rule out cardiac amyloidosis which showed radiotracer activity in the apex and inferior apical wall. A cardiac biopsy was performed and showed the above findings. What do you think, is it straightforward amyloidosis?

Of course not! We're going to the zoo to find some zebras today! These images show myocytes with cytoplasmic vacuoles, patchy cardiomyocyte hypertrophy, and a Congo Red stain negative for amyloid deposition. On electron microscopy there are concentric lamellar structures (myeloid or zebra 🦓 bodies) and scattered curvilinear bodies. Zebra bodies are classically associated with Fabry disease but they can occur in other settings as well, including rheumatoid arthritis, drug-induced phospholipidosis, and other diseases. In this patient, the finding is most likely a result of hydroxychloroquine induced cardiotoxicity. 💔🦓

November! 🦃 This month flew by with conferences and sightseeing around the country, our annual department Thanksgiving potluck, happy hour on the water, new babies (human & canine!), renaissance fair fun, cytopathology shenanigans, and much more! Hello it's time for our November case of the month! This month we have a 24 year old woman presenting with a cystic lesion of the left o***y on ultrasound undergoing a unilateral salpingo-oophorectomy. The histology findings are attached above, what do you think? Did blocks get switched cause it doesn’t look like o***y!​ 🫢😱😱😱😬😳

This is a mature cystic teratoma! These tumors display growth of different mature tissues like skin, hair,
teeth, cartilage, etc. But, some cases show more uncommon findings like this one! Though skin with
adnexal structures are present we also see a focus of thyroid (Image 2 and 3) along with neuroglial tissue
(image 3 and 4). These findings are important as the patient may need thyroid function tests due to
potential ectopic production of thyroid hormone. In addition, screening for the presence of anti-NMDA
encephalitis is needed as some patients with brain tissue within teratomas will develop an autoimmune
condition affecting their own native brain tissue. Thanks for reading!​
