We will try to occasionally address questions that are often asked by our patients. Please keep in mind, all care in our practice is individualized. We keep our patient population intentionally manageable to have time to discuss these issues with you. Today’s focus will be on vaginal delivery after prior cesarean section.

TOLAC

✅ TOLAC is generally considered safe for many patients, especially when:
• You’ve had one prior low-transverse (“bikini line”) incision
• No history of uterine rupture
• No other medical or pregnancy-related reasons a vaginal delivery isn’t recommended
• Delivery occurs in a hospital that can provide emergency C-section if needed

⚠️ When is TOLAC NOT recommended?
TOLAC is typically contraindicated with:
• Prior classical (vertical) uterine incision or T-shaped incision
• Prior uterine rupture
• Certain prior uterine surgeries (including some full-thickness myomectomies)
• Any condition where vaginal delivery is not advised (example: placenta previa)

VBAC / TOLAC + Uterine Wall Thickness: What the Evidence Shows

If you’ve had a prior C-section, a TOLAC (trial of labor after cesarean) may be an option. One factor sometimes discussed is lower uterine segment (LUS) thickness on late-pregnancy ultrasound.

📏 What studies show:
Research consistently finds that a thinner LUS is linked with higher rates of scar defect/uterine rupture. Several studies and protocols identify very low thickness (around ~2.0–2.3 mm) as the highest-risk range, and newer data suggest risk may increase even below ~3.0 mm in some populations.

⚠️ Important nuance:
Ultrasound thickness is not a perfect “yes/no” test—results depend on how it’s measured and it should be interpreted alongside your history (type of uterine incision, prior rupture, prior uterine surgery, number of prior C-sections, induction needs, etc.).

🤍 Our approach: individualized counseling—balancing your goals with the safest plan for you and baby, in a setting prepared for urgent intervention if needed.

Acetaminophen remains a safe, trusted option for pain relief during pregnancy. Despite recent unfounded claims, there’s no clear evidence linking prudent use to issues with fetal development. ACOG's guidance remains the same. When pain relief is needed during pregnancy, acetaminophen should be used in moderation, and after consulting your doctor.

Menopause and perimenopause is a challenging and complex time for women. The desire for optimizing health and well being during this time leads many women to falling into traps of purchasing products or paying for evaluations that are unnecessary and expensive. We are often asked about resources and books. This book is at the top of the list. Dr Gunter is seen as the best among the top OB/Gyns in the country. She sells no supplements nor is she paid by any lab companies, etc.
