Good Afternoon, we are officially live with Valley Health. Due to the transition, the registration and check in process may take longer than normal. To reduce this time, our staff may be reaching out ahead of your visit to collect necessary information by phone. When you arrive in person, please bring your ID and insurance card. We apologize in advance for the increased wait times and appreciate your patience and understanding.

‼️ ‼️ We are excited for the new transition with Valley Health Systems taking place this Friday, March 1st.

As you come in for your appointment your child will be registered as a new patient. To help things run smoothly we ask that you arrive at least 15 minutes prior to your appointment time.
You must bring a physical copy of your current INSURANCE CARD and PHOTO ID.

You may have already began receiving a text or email to sign up with the MyChart Patient Portal. We would like to ask that all patients sign up for this as it will give you access to records, results, patient messaging, prescription request and schedule appointments.
****If your child is 13 years or older you will need to see the front desk receptionist for patient portal access

With the transition to Valley Health we will be switching to a new phone system. This transition will start this evening through tomorrow morning. When calling you should not notice any change. If you do not hear back from the provider on call this evening with in an hour, please call back. We ask that you please be patient with us as we work through the process. Thank you for your understanding.
