Submit an inquiry today to schedule your child’s ESA+ Grant Re-Evaluation. To determine if your student’s re-evaluation date is approaching:
1. Find your student’s current IEP Eligibility Determination Form.
2. On the last page there should be a list of people who attended the meeting and the date.
3. Find the date of the meeting in the “Date” column and add three to the year.
