Our organization has consolidated with Vaya Health, all other counties that we served have realigned with a new LME/MCO. You can learn more at the NC Department of Health and Human Services website.

📍 Cabarrus, Davie, Forsyth, Stanly, and Union Counties have transitioned to Partners Health Management.

📍 Davidson and Rockingham Counties, you have realigned to Sandhills Center.

📍 Halifax County, you have realigned to Trillium Health Resources.

📍 Warren County, you have realigned to Eastpointe.

📍 Mecklenburg and Orange Counties, you have realigned to Alliance Health.
