Equity Health: Bringing Compassionate Care Home

Michelle Schmerge, DNP, shares how Equity Health, a VIA Health Partners company, is helping people “live longer, better.”

From disease and medication management to minor clinic procedures — all provided in the comfort of your home — Equity Health home-based primary care services are designed to make life easier for patients and caregivers alike.

For many families, the simple act of preparing a loved one for a doctor’s visit can become overwhelming. That’s where Equity Health comes in — helping ensure medications are filled and taken properly, conditions are managed proactively, and care happens where it feels safest: at home.

If getting to appointments has become difficult or if you’d like to learn how our team can help, reach out to Equity Health, a VIA Health Partners company.
📞 Call us or learn more at 704.489.3440 or www.viahp.org/equity-health

Michelle was asked back to WSIC News's Good Morning LKN to talk more about the challenges for those in our community who are older and need varying levels of care.

The importance of preventative care and people living longer was discussed - how to take care of yourself so that as you age you're able to maintain independence, quality of life, and dignity while minimizing health complications that could require more intensive care down the road.

Being nominated for a DAISY Award is a tremendous honor that exemplifies the kind of care a nurse provides to her patients. So, when Joyce Davis was nominated not once… not twice… but three separate times by two different patient families … all we could say was WOW!
