Blue Ridge Care Connection for Children is a small office that partners with UVA and the Virginia Department of Health to provide free care coordination services for children with special healthcare needs. We gather case managers, medical providers, educators, and family members together to plan and manage a plan of care specific to each child. We strive to:
-Build strong, caring relationships with each child and family
-Provide one-on-one, individualized attention
-Gain full understanding of the medical, emotional, educational, and social needs of each child and family
-Give comprehensive and culturally appropriate care

We Assist With:
-Access to community resources
-Access to medical specialty care
-Ability to pay for items not covered by a child's insurance
-Communication with schools, payers, providers, and community services
-Healthcare payment and insurance options
-Medical visits
-School challenges
-Social supports
-Social transitions
-Understanding of health insurance benefits
