As part of our ongoing effort to enhance the quality of care and improve your overall experience with us we are pleased to inform you that our practice is transferring to a new Electronic Health Record (EHR) system, Elation.

What This Means for You
With the new EHR system, you will have access to a user-friendly platform called the Patient Passport, which offers several benefits

Appointments: Easily view and manage your upcoming appointments.

Forms: Access and complete necessary forms ahead of your visits.

Medication Lists: Keep track of your current medications.

Communication: Enjoy a streamlined way to communicate with our office.

Additionally, you will have the option to sign up for appointment reminders, ensuring you never miss an important date.

Next Steps
You will receive an email and text message inviting you to sign up for the Patient Passport. The process is simple and will provide you with immediate access to these new features.

We are excited about this transition and confident that the Elation system will significantly enhance your experience with us. If you have any questions or need assistance during this transition, please do not hesitate to contact our office.

In observance of the upcoming holiday weekend, we want to inform you of our adjusted hours. On July 3rd, our office will close at 3:30 PM. We will be closed on July 4th in observance of Independence Day. Our regular hours will resume on July 7th.
