Too many times I see providers practicing implied consent, which is when your provider doesn't fully explain the risks and the benefits of what they are doing. they simply assume they have your permission based on circumstance since you are under their care. And they tell you what they are going to do instead of asking.

Whenever a procedure, exam, or medical treatment is recommended to you, your provider is responsible for explaining the benefits, risks, and any further information you need to fully understand what they are presenting in order to make an informed decision without coercion. And you have a choice to accept or decline that recommendation. That is informed consent.

Today is International Hyperemesis Gravidarum (HG) Awareness Day. As some of you may know, I suffered from HG. With all three I experienced extreme nausea and vomiting. I couldn't keep ANYTHING down and was constantly throwing up bile. None of the medications worked and every single day I struggled to get out of bed.
Even though my symptoms were the same, each experience was different. With my oldest I had no clue why I couldn't stop vomiting. Everyone kept telling me not to worry. That it was normal morning sickness, but my intuition kept telling me that THIS wasn't normal. After countless trips to the ER for dehydration, I'm glad I trusted my gut and called my OB. After additional testing confirmed I had HG, he then decided to put me on home IV therapy.

With my 2nd, I started seeing a new OB my friend referred me to. Because of my experience with my first, I can confidently say this provider did not take my symptoms or concerns seriously at all. She kept brushing me off and made it seem like I was experiencing normal "morning sickness". It wasn't until I switched providers that I began to get the treatment I needed and deserved.

With my 3rd, my HG was so severe that I had to be hospitalized until I was able to get a PICC line to begin home IV therapy. That time I spent in the hospital was extremely hard for me because I was alone and missing my family. With my first, my husband was right by my side every time I went to the hospital, but this time around it was harder for him to be there every moment with work, taking care of the first two, and handling our daughter's busy schedules. I missed my daughters very first christmas recital and other precious moments which led to me experiencing depression. I dealt with the anxiety and stress due to my employer constantly threatening to terminate my employment while on disability if I didn't return to work. I even needed my PICC line to be removed prematurely, because I was showing early signs of a potential risk of infection in my bloodstream. Eventually my HG improved with all of my pregnancies, but I still had symptoms up until the days I gave birth to my beautiful children(Rest in Comments)

When a care provider shows negative expressions, emotions, or remarks when reviewing your birth plan this is a huge red flag. A provider that is not willing to take your ideas, preferences, and questions into consideration could be an indication that he/she will not support your vision for your birth when you go into labor. If you feel like you have to either give in or fight for what you desire to happen then run for the hills. You have a right to be involved in your care and reviewing your birth plan is part of that process. This is why I encourage everyone that is expecting to interview your provider in the very beginning. Make sure they understand your vision for your birth and you feel comfortable and confident that they will respect your wishes. And if not switch and find a provider that will. THAT’S YOUR RIGHT!
