Our dedicated care management team includes health and human service professionals who work with aging adults, people with disabilities, and their caregivers to facilitate an individual’s health and well-being.
Increasingly, adult children are taking on the responsibilities of caring for aging parents. Known as the "Sandwich Generation," these individuals are often caught between the demands of caring for aging parents and rearing children of their own.
Aging adults, those with disabilities, and their family caregivers often face difficult and stressful challenges that can result in caregiver burnout, incomplete management of the aging parent’s medical care; medication regimen; financial assets and banking; and more. Families want to honor a parent’s desire to "age in place" while balancing safety and security concerns. Today more than ever, families are spread out geographically, sometimes leaving few or no family members to assist an aging parent who lives alone.

Our Care Managers conduct in-depth assessments, develop individualized service plans and recommendations, coordinate services, act as liaisons to health care providers and insurers, and monitor services to ensure that a person’s needs and goals are being met. Staff is knowledgeable about resources, and work closely with family and caregivers to identify community supports; reduce stress; and help make the caregiving process as positive as possible. Care Management begins with a comprehensive assessment to determine an individual's unique strengths and needs as they relate to a variety of issues, including:
Self-care abilities
General health & cognitive functional status
Medication management
Fall risk status
Driving & transportation concerns
Social supports and circumstances
Home & personal safety status
Financial resources & entitlements
End-of-life planning
We conduct a face-to-face assessment, taking note of the medical, social, financial, legal and spiritual circumstances. Based upon this assessment, we work with the individual and his or her family/professional care team to develop an individualized service plan reflecting short-term and long-term goals. Assessments can also be conducted via telephone as needed. Assessments usually take one to two hours, depending on the individual and his or her circumstances.
Rapid Intake
Sometimes, an unexpected change to a loved one’s financial, medical, or housing circumstances requires a family to act swiftly to ensure the loved one’s well-being. In these circumstances, Tidewater Elder Services can perform a brief intake (in person or by phone) to avoid delay in addressing any crisis. Once the situation is stable, a complete assessment will be scheduled.
