Craniocervical junction (CCJ) instability occurs when there is excessive movement or misalignment between the first (C1, atlas) and second (C2, axis) cervical vertebrae.

This condition can result from trauma (like whiplash), congenital issues, degenerative diseases, or inflammatory conditions.

When the CCJ is unstable, it can place pressure on the brainstem, spinal cord, and surrounding blood vessels, leading to symptoms such as chronic neck pain, headaches, dizziness, difficulty swallowing, and neurological problems like numbness or weakness.

The condition is often difficult to diagnose because its symptoms can overlap with other neurological or musculoskeletal disorders.
