If you've had the vaccine can you get an antibody test and see if you've also had COVID? This is a question I get from time to time. And the answer is it depends on the test. Some tests can tell you, some cannot. Let's dive into this one.

First, what is an antibody and why do we measure it? An antibody is what your innate immune system makes when it is exposed to a foreign invader (in the case of COVID it's a virus) or to the vaccine. It helps your body neutralize and fight the virus. We measure it because it can tell us if you've been exposed to the virus or had the vaccine.

The picture below is of someone who has had the vaccine but has NOT had COVID. How do we know? Because they have antibodies ONLY to the spike protein and receptor binding domain. This is the pattern of immunity generated by the vaccine. Notice this person do NOT have antibodies to the nucleoprotein. Antibodies to the nucleoprotein cannot be generated by the vaccine because the vaccine does not code for nucleoproteins, only spike proteins. A person who had COVID would have antibodies to all 4 antigens (S1, S2, receptor binding domain, AND nucleoprotein).

Not all tests will give you this information. Many widely available tests only check for antibodies to the spike protein. This CANNOT tell the difference between infection or vaccine induced immunity.

This example is from a test we run here at the office and is done with a simple finger stick.
