Our purpose is to care for life altering illness and catastrophic injuries with a team approach. We are be able to immediately pull in the right expertise and enable a proactive, collaborative approach—ultimately focused on the sick and injured health and well being—while simultaneously working to control costs for the payer. Our medical case management, rehabilitation nurse case manager, and home care team will reduces the volatility by correcting the trajectory of medical utilization, improving clinical results and lowering expenses. Our catastrophic complex care teams can provide care coordination, clinical oversight, and case stewardship.The first consideration is injury type. The team then works to identify if comorbidities exist, and if they might complicate the injury, the injured worker’s medical needs, or the healing and recovery process. As nurses we are able to identify comorbidities and have an understanding of how these factors adversely affect claims’ outcomes. Comorbidity factors may include diabetes, heart disease, high blood pressure, poor circulation, obesity, and the age of the injured worker. By identifying these factors, the team can anticipate risks and plan to monitor and address issues early. Ultimately, this helps to better control the direction of the claim. For each injury, the catastrophic claims team would provide ongoing coordination of medical services to meet the injured worker’s needs, including home care,medical equipment, devices, supplies, and other specialized services. When necessary, team members would communicate directly with physicians to coordinate care. When our team is notified at the initial stages our assessment our case manager will visit the claimant’s home, interview family members, and ask extensive questions to arrive at the best overall recommendation. We take every measure to meet the injured worker’s long-term needs. We strive to hear the voice of the injured worker.It can be difficult to gauge when and which treatments are working and what to do next. Some conditions are easy to monitor, but others do not have clear clinical markers. Our staff is easily able to capture direct claimant input regarding mental health, physical functioning, and overall quality of life. By gathering this feedback with physician notes, case management reports, and claims data, the team can gain a broader picture to monitor progress and adjust care to improve outcomes.
