Your zip code shouldn't determine your lifespan. We are proud to partner with Live Healthy Chicago to close the 12-year life expectancy gap on the West Side by providing critical resources for hypertension management.
Through this collaboration, eligible residents can receive free blood pressure monitors and personalized training from Community Health Workers. By putting these tools directly into your hands, we are empowering our neighbors to track their heart health from the comfort of home.
Together, we are turning the tide on health equity through direct clinical support and community care.
