Thresholds Health, a division of Thresholds providing primary care services, has officially transitioned operations to PCC Community Wellness Center. PCC is a trusted Federally Qualified Health Center with a strong track record of delivering affordable, community-based healthcare across Chicagoland.

This transition allows the clinic to continue serving patients at its longtime West Side location, 5801 W Corcoran Pl, while also creating opportunities to expand services with PCC’s expertise and resources. For current patients, care continues with minimal disruptions or changes to care plans.

Thresholds Health was proud to be part of the Juneteenth Grand Opening of the Aspire Center for Workforce Innovation in Chicago's Austin neighborhood. It was a beautiful and inspirational day spent with our community, including local and city leaders. This will serve as a new resource and infrastructure for our community, providing vital tools to help residents thrive!

The heat wave continues!

All Community Service Centers and Senior Centers will be open as Cooling Centers from Monday through Friday this week, including our 15 satellite Senior Centers: chicago.gov/seniors

Residents looking for additional Cooling Center options this weekend can find relief in one of the City’s 79 Chicago Public Library locations, seven City Colleges of Chicago main campus locations, 43 Chicago Park District fieldhouses, as well as pools and 143 splash pads located throughout the city — visit www.chicagoparkdistrict.com for more information. The Chicago Police Department’s 22 district stations are also available 24 hours for relief from the heat and to be connected with shelter.

For shelter placement, to find the nearest cooling center, and for well-being checks for seniors and unhoused neighbors, call 3-1-1.

We were invited by the Institute for Nonviolence Chicago "Embracing Humanity" event in honor of , which brought together thought leaders, advocates, and community members, such as Chicago CRED, Alderman Emma Mitts and IL State Senator Kimberly Lightford, to spotlight efforts in community violence intervention through public health, policy reform, and direct outreach.

Thresholds Health is partnering with Roseland Community Hospital Mobile Mammogram Van to provide breast cancer screenings for our community. On November 13th and December 2nd, the van will be outside of Thresholds Health from 9am - 2pm.

While you're there, meet our staff and learn more about the services Thresholds Health can provide to keep you well, including vaccines (flu and COVID-19), physical exams, mental health treatment and more!
