The One Big Beautiful Bill Act (OBBBA), signed into law by President Trump on July 4, 2025, is set to significantly shift the U.S. health care landscape – particularly toward direct-pay and cash-based models.
With millions expected to lose Medicai or ACA exchange coverage due to stricter eligibility, reduced subsidies, and rising premiums, a growing number of patients will turn to cash pay or out-of-pocket options for prescriptions.
This is prompting drug manufacturers to launch direct-to-patient sales programs—such as discounts on certain drugs —bypassing PBMs entirely.
For community pharmacies, this opens opportunities to serve uninsured patients through cash-pay memberships, generics, and possibly partnerships with manufacturers. As traditional reimbursement models erode, pharmacies are urged to adapt quickly to this emerging direct-pay ecosystem, which is gaining momentum and public favor.
