In the new Clinical Foundations for the Primary Care PT part 1, one of the things I was very intentional about was drawing awareness to the important contextual factors associated with the initial patient interview - like consistent flow, the verbal and physical language used, the space you're in and how you respond to the patient's story. AND the efficiency associated with it. Oftentimes, this is an area that separates average from expert level practice... which is why we focus on it!

"I'll never finish 18 hours online."

Yes you will, babe! Don't sweat 💦!

Primary care PT is not for every PT.

I posed a question on a few social media accounts yesterday: "is primary care PT for every PT?" And I promised to share my opinion after the polls. Note the photos here with the various poll results.

My opinion is consistent with the majority, that primary care is not for every PT.

My opinion may surprise some people, especially since my career and business revolve around the education, mentorship and development of this practice area, but my heavy time and knowledge investment in this area over the past decade has led me to believe the following:

1) Many (the vast majority of) PTs are comfortable in traditional rehab. Many aren't ready, nor interested in adapting their practice patterns to fit the mold of primary care. This is okay! We will always need traditional "rehab" PTs. Tons of post-op and trauma to go around.

2) Many excellent PTs are focused specialists (neuro, ortho, pelvic, CVP, peds, etc), which we absolutely need! When you're an expert in one area, you are doing a great service to the patients who need your service. Nothing should change with that. We need focused specialists.

There's room for all of it. Primary care PT is not for every PT.

Direct to employer healthcare models are the answer to everyone’s hopes and prayers about health service payment reform. So now my question is... why in the world aren’t more PTs doing this? We have a movement of clinicians (not just PTs) leaving healthcare altogether due to burnout because the admin burden and productivity requirements are unsustainable. American consumer/employees/patients (call them what you will) are NOT using any health benefits even when they’re sick or injured because of high deductible plans that employers are still pouring a ton of money into... why isn’t this more mainstream for PT?
