Among patients undergoing elective midline , continuous small-bites fascial closure significantly reduced the long-term incidence and size of incisional compared with large-bites closure.

At 13 years, the cumulative incidence of incisional was 34% in the small-bites group vs 49% in the large-bites group, with clinically significant hernias (>20 mm) also halved. Hernias that developed after small-bites closure were smaller and progressed more slowly, supporting small-bites as the standard practice for abdominal wall closure.
