Thanks for coming in Sofia! Sofia is actually our neighbor and is a fellow business owner as well. She owns a meal prep service and makes different meals each week that are fantastic. The meals are labeled with calories and macros and make life so much easier if you’re busy. I use the service myself and it’s the only meal prep company I’ve ever used where the meals are actually delicious 😂

Now, a quick insight about this visit. While trying to manipulate Sofias neck, I could feel she was guarding a bit during the thrust of the movement. I don’t thrust very hard and if the manipulation doesn’t work once and it isn’t there, I don’t force it. If you’ve done enough of them, you’ll know when to stop trying if the first attempt doesn’t feel right. That said, we opted for soft tissue therapy because it practically does the same thing, it just takes a bit longer. There’s more than one way to provide a therapeutic effect 🤝

Here’s a session with my patient seeking treatment for his painful shoulder due to adhesive capsulitis and RCRSP.

Some takeaways from the video:
1. My “flow” for care is dependent on the flow of the conversation and the position of the patient. When the patient was on his back, we did all the things we could do when he was in that position. When it comes to the order of MT and exercise, which comes before or after is probably inconsequential so my treatment blends both together.
2. Throughout treatment I’m always asking questions. For example, the patient told me pressing movements were fine, but I wanted to see for myself so I checked them. This brought up a question the patient had about pressing being “safe”. I assured him that pressing was safe so long as he doesn’t have any long lasting debilitating pain afterwards and reassured him so that he can stay active.
3. Your rehab does not need to be long or complicated. It just has to work. I’ve treated Greg for the same issue before, so we have some idea of what movements give him relief and more ROM. In the video he goes on to say he did some movements in the HEP he was given and that they helped him retain some ROM and strength, but later on (not on video) he admits he probably hasn’t been doing them as much as he should be because he is often busy and forgets.

4. Yes, I said “loosen up” the tissue. Greg knows through previous discussions that I am not loosening his muscles physically by rolling them out like pizza dough so don’t call the nocebo police it’ll all be okay.

30 min manual therapy session for my client Ang distilled down to 5 minutes. Also shown are some isometrics of the shoulder and the neck.

We do a lot of manual therapy interventions for Ang for the simple reason that he feels each one has a cumulative effect on his health and helps him to perform better in the gym. One reason I can do this for Ang is because my fees are a flat rate, so getting “more” doesn’t put Ang at a financial disadvantage and because he enjoys all the bells and whistles and we have the time, why not do what he feels helps him?

Not shown here during this encounter is the brief (15 min) coaching interaction Ang and I had trying to narrow down the cause of his neck exacerbation. We narrowed it down to a few things. First is his very frequent implementation of face pulls with high intensity. Through trial and error of taking out and supplementing certain movements, we figured out that chest supported and seated variations of back movements such as t-bar rows and reverse flys don’t exacerbate his neck pain, but face pulls do even when modifying form, frequency, tempo, load and intensity. The movement also wasn’t very important to Ang so finding a variation to stimulate hypertrophy much the same way didn’t cause him any grief and we can arguably more easily load the chest supported/seated movements anyways.

Ang also has Achilles tendinopathy which he was given a HEP for. We don’t need to overload Ang with rehab, he works out 6 times a week. This is the perfect example of a patient who simply needs some reassurance, some temporary pain interruption and some exercise selection guidance so he can stay out of debilitating pain and keep himself healthy

Enjoy some chiropractic spinal manipulation ASMR with me and my chronic neck pain patient.

I’ve said this before, but I think somewhere down the road while I was being critical of the grandiose claims practitioners have historically made about manual therapy, I was lumped into the anti-manual therapy club. While I’ve heavily critiqued claims made by manual therapists, that doesn’t mean I dislike the intervention. You can dislike the stigma and taboo around something because it creates a false narrative, but that does not mean you hate the subject of contention itself.

I utilize manual therapy on nearly every patient. I have remote rehab clients I never meet in person, I have in-person clients who just want rehab, I have clients who specifically do not want rehab and feel that manual therapy is the best use of their time. It’s important for us to realize that none of these options are wrong. To look down on patients who only want manual therapy is self-righteous and arrogant. We do not decide how the patient receives care, we make the suggestion based on best evidence and then THEY decide what they want. To coerce patients into doing an 8 week rehab program when they can cope with their pain fairly well on their own and just want some manual therapy here and again is to be no better than a manual therapy charlatan.

It’s your job to educate the patient and meet them where they’re at. This patient in particular has done rehab with me before, he will probably do rehab again with me at some point, but today was a manual therapy day and that’s totally fine too.
