You have just completed a case of sudden, unwitnessed death in an elderly man. He has been treated recently for a new-onset arrhythmia and was scheduled for a pacemaker placement. The autopsy showed massive cardiomegaly from hypertension and also an 85% blockage in the right coronary artery. There is a great deal of fluid (edema) in the lungs. You are sitting down to complete a death certificate. What will you put for the immediate cause of death when you complete the death certificate?
A. Undetermined.
B. Pending (you want to review microscopy to see if there was any myocardial injury).

You are working on a case report and have some additional questions for the family to help you figure out the case. The report is not yet signed out. While speaking to the family, they ask you to review some of the findings you wrote on the death certificate related to the cause of death. The best response is to:
A. Give your best answer to their questions now, because it's considerate and they want to know.
B. Listen to the question but reserve your response till after the case is signed out.
C. Tell the family you can't discuss anything on the death certificate, but that you can take the question once the report is complete.
D. Explain that the original death certificate stops being valid once the report is complete, so you prefer to wait till then.

A family calls you and says their loved one died one week after a surgery. It was a scheduled bowel resection for cancer. They were told the surgery went fine, but when the patient woke up, he could not maintain his blood pressure. He was found to have had a heart attack. They are requesting an autopsy. Which of these is probably reasonable to consider?
