This book is titled A Practical Guide to Hospital Readmission Reduction because “practical” has become my focus. My goal is to give case managers a “one stop shop” for education and resources to “make it happen”. Much like my first book “A Practical Guide to Acute Care Case Management”, this book is your tabletop resource for knowledge on the Hospital Readmission Reduction Program and reviews evidence-based practices and tools you can use in your real-life practice.
