It is with heavy hearts that we share the news of Dr. Rabi Sulayman’s passing.
Dr. Sulayman was with our practice for 15 plus years and loved every minute of it. We will forever miss him walking through our doors with his coffee in hand, huge smile on face but most importantly the love he had for each and everyone of his patients. We are truly heartbroken over this and ask that you all keep his wife, children and grandchildren in your thoughts and prayers. He has touched the lives of everyone who knew him and we are all better people for knowing him.

Good afternoon to our wonderful families!

We hope everyone is staying safe and continuing to social distance, as hard as it is.

We have some good news!

The office will be open on Monday’s and Thursday’s for wellness visits for right now. We are reaching out to patients who have called over the past couple months but if your child needs to come in for a check-up, school physical, immunizations, please call us to schedule that appointment.

Vaccine/DHS mandated forms are needed for our babies 2 months through 30 months,
children entering Pre-K, Kindergarten, 6th grade, High School, Senior Year and college. Remember Sport physicals are due every year for those who play sports, both grammar and high school students.

Hello-

We wanted to let everyone know that we miss each and every one of our families and cannot wait to get back into the office. We are currently working on trying to organize a vaccination day for our babies/toddlers. Once that has been implemented we will reach out by phone, text and Facebook. We are hoping to be back by June, but know that if you need anything in the meantime, we are here in anyway possible.

We would also like to take a moment to thank each and everyone of you that sent your well wishes by praying, calling, texting, sending a get well card and/or flowers to Dr. John. Dr. John is over the moon by the outpour of love from you all. Dr. John is doing incredible, she is truly an avenger and with each day she is getting stronger and more eager to return to you all.

We hope everyone is staying both healthy and safe during these trying times. Again, if you should need anything, please reach out, we are here.

Update on COVID-19

Due to the evolving precautions and recommendations by the CDC and community health leaders, EFFECTIVE 3/17/2020, no one other than ONE parent will be allowed to come with your child to their appointment. Please leave all other children at home, if they do not have an appointment, they will not be able to come into office. Everyone that comes into office will be asked to put on a mask, this is for both the safety of yourselves and others. As of right now, we will be pushing both school and sports physicals until the end of May but will continue to update you as new information keeps coming. We understand all of this will be an inconvenience. We, at Annita John, M.D.,P.C., are taking every precaution we can to reduce the likelihood of exposure to COVID-19, however we still need your help and cooperation to reduce this risk. During this time, if you need to cancel or reschedule your appointment, NO MISSED APPOINTMENT FEES will be assessed. We will work diligently to accommodate appointments being rescheduled.

We thank you for your continued patience and understanding as we navigate through these uncharted waters together. And as always, keep washing your hands!

Our deepest and sincere condolences to one of our families, the Mayfield’s. We are truly heartbroken over this news. Please keep this family in your thoughts and prayers, no parent should ever have to bury their child.

The body of Henry Mayfield Jr., the Hillcrest High School alum who was killed in an attack in Kenya, will be escorted home Jan. 15.

To our patients- We just wanted to let all know that we got hit pretty bad from the storm and the entire office has to be renovated. We WILL be seeing patients at the north pavilion at Little Company of Mary until Saturday and then we will be working out of Dr. Venkataraman’s office located at 10458 South Pulaski. If you need to schedule an appointment, referrals, questions, etc you can reach us our number 773-238-1616 if you cannot get through you can call my cell at 773-419-1076. We are so sorry for the inconvenience but will do all we can to accommodate our patients.

Dr. Annita John and Jirah! Dr. John has been taking care of him since birth and now he will be graduating our practice to move on to bigger and better things. He has received a scholarship to play college football! Good luck, Jirah!
