At CommunityHealth, we understand the tremendous needs of the uninsured population in Chicago– a population that continues to grow in today’s economic climate. Thanks to an innovative and highly effective model supported by volunteerism, philanthropy and partnerships, CommunityHealth will provide 30,000 medical and dental visits at our West Town location this year for over 9,000 patients. physicians, nurses, dentists, counselors, donors (individuals, foundations, corporations, etc.) and various institutional (e.g. hospitals, medical schools, universities/graduate programs) and community partners recognize the critical role that CommunityHealth is playing in the health of those in our society who would otherwise go without care they need. While primary care is the cornerstone of our mission, CommunityHealth is a full service, appointment-based medical home providing more than 20 specialty services (from cardiology to ultrasound), health education, case management and social work (available both on-site and through formal referral partners). CommunityHealth attended the CARES Community Center Groundbreaking Ceremony yesterday to support our microsite partners, Onward Neighborhood House

In the CARES Center, CommunityHealth at Onward House will expand to multiple exam rooms, a waiting area, a permanent behavioral health office, a kitchen, and become co-located with several other health organizations.

Congresswoman Delia Ramierz, Representative Eva-Dina Delgado, Representative Will Guzzardi, Senator Omar Aquino, Senator Graciela Guzman, Alderman Ruth Cruz, staffer from Alderman Felix Cardona's office, along with other Chicago leaders, were in attendance.

The new community center will impact the well-being of the Belmont Cragin neighborhood and beyond! CommunityHealth is thrilled to be part of it, and the buildout of our clinic space in the new building is made possible by Lundbeck.

Our outreach worker Daniel held a Healthy Homes workshop in Spanish at our Enlace microsite today! We had 30 attendees, and our partners from Elevate were there as well to answer any questions patients may have about the home renovations. Attendees received goodie bags with home essentials!

Our next Healthy Homes workshop will be this Saturday, March 25th from 10 AM - 12 PM at Enlace.

From 6:30pm-9:00pm, CommunityHealth will be partnering with Patagonia to host “Community in Action,” where every dollar raised supports CommunityHealth’s work providing free, high-quality health care to the uninsured in Chicago.

At the event, you will learn more about the work CommunityHealth and the Associate Board does, all while enjoying live music, raffle prizes, and Patagonia discounts!

Earlier this week, CommunityHealth and our Health Education team members Zeneida and Corali were awarded a Certificate of Recognition by Richard J. Daley College at the Arturo Velasquez Institute!

The award was given in appreciation for the collaboration, partnership and commitment to supporting the Community Health Worker Program.
