***ADHD and Comprehensive Psychological Assessment/Diagnostic Clarification. MAY BE COVERED BY Aetna PPO, BCBS PPO, BLUE CHOICE, AND BLUE SELECT**

DCFS involved clients: Dr. Tiffany is a DCFS contract Psychologist for individuals 13 and up. Comprehensive Psychological Assessment/Diagnostic Clarification might be covered by DCFS. You must have completed and approved a CFS 417 and a CFS 417-C along with supporting documentation to make a referral.

What we offer:
*Attention Deficit Hyperactivity Disorder (ADHD) Assessment - A comprehensive evaluation that examines behavioral, emotional, and cognitive patterns to determine if they align with the diagnostic criteria for Attention Deficit/Hyperactivity Disorder.

*Comprehensive Psychological Assessment/Diagnostic Clarification - A thorough and comprehensive evaluation of an individual's psychological, social, mental, intellectual, cognitive, personality, and emotional function to form a clinical conceptualization and produces treatment recommendations.

*Child Custody Evaluation - A Child Custody Evaluation is psychological investigation into a child's home, family environment, background, and other circumstances of a child, which is used by the court system to determine child custody and access to the child. Child Custody Evaluations focus on the psychological best interest of the child.

*Domestic Violence Assessment - During a domestic violence assessment individuals will be interviewed in relation to their history of domestic violence. Assessment will provide domestic violence treatment recommendations.

*FOID Reinstatement Assessment- Evaluates an individual's mental health, behavioral history, and risk factors to determine their suitability for restoring firearm ownership privileges.

*Parenting Capacity Evaluation - A Parenting Capacity Evaluation is a thorough and comprehensive evaluation of a parent's or caregiver's risk for neglect and/or abuse.

*Substance Abuse Assessment - A comprehensive analysis of an individual's substance use history and current substance use to provide treatment recommendations and level of care.

I am now a contract psychologist for DCFS!! If you have clients, 13 years old and up, waiting for psychological assessments, I can get them in fairly soon. You'll need to complete, and receive approval, the CFS 417 referral form with the supporting documents. Reach out to me before you submit it so I can give you my provider information.
