Esperanza Health Centers Since 2004, Esperanza has been delivering care of the highest quality regardless of immigration status, insurance status, or ability to pay.

Esperanza provides bilingual, high quality primary care, behavioral health, and wellness services to the community, regardless of immigration status, insurance status, or ability to pay. Esperanza Health Centers is a Federally Qualified Health Center that operates five primary care clinics on Chicago’s Southwest Side for over 35,000 patients per year. Through the provision of a full range of bilingual, culturally appropriate primary and specialty care, behavioral health, and wellness services, we are improving health equity and reducing barriers to care for the residents of the medically underserved, primarily Latino communities we serve.

Last night, our residents and provider team proudly represented Esperanza at a dinner hosted by UNITY Screen.

The conversation focused on how prenatal genetic screening can help detect and treat conditions early, leading to better outcomes for both mom and baby.

We’re especially proud that Esperanza providers made up more than half of the clinicians in attendance, reflecting our commitment to delivering the highest quality care.

The 340B Program is vital for our patients. It helps ensure that individuals and families, especially those who are uninsured or underinsured, can get the medications they need without having to choose between their health and basic necessities like food or rent.

At Esperanza Health Centers, we’ve been working alongside community partners and organizations across Illinois to make sure 340B is protected and brought to a vote. This program is not just policy, it’s a lifeline for our community.

Did you know that there is a program called 340B that allows many of our uninsured and underinsured patients to get their medication at a cost they can afford? Because of 340B, many don’t have to choose between their life-saving medication and food or rent.

Sadly, the pharmaceutical industry has been trying to undermine this program for years, and now they are in Springfield trying to stop a bill that would actually protect your access to more affordable medication. The only way we can win is if our legislators hear from us and today, Esperanza is asking you for help.

Our Community Engagement team is working toward a goal of distributing over 300 bags of food, and bags are still available for anyone who may need one while supplies last. We are also providing 200 bags of hygiene products, including items like shampoo and toothpaste, generously supplied by .

Our Patient Benefit Navigators are also on-site, offering support and helping individuals better understand and access their healthcare benefits.

We are grateful to everyone who has stopped by to connect with us and learn more about the resources available. We look forward to continuing to serve our community throughout the day.

Esperanza is excited to partner with Delfina Care to offer additional support for patients during pregnancy, including nutrition.

Through Delfina’s nutrition support program, Esperanza patients can access free virtual meetings with registered dietitians who provide personalized guidance to help support a healthy pregnancy and baby. Patients can receive one on one advice, participate in group sessions, and learn practical ways to make healthy food choices during pregnancy and postpartum. Through the Delfina app, patients can receive virtual support, track health information, learn more about their pregnancy, and get tips on nutrition and overall wellness.

Esperanza patients can download the Delfina app to get started and take advantage of this free nutrition support.


Esperanza se complace en colaborar con Delfina Care para ofrecer apoyo adicional a las pacientes durante el embarazo, incluyendo nutrición.

During Women’s History Month, we recognize the many contributions women have made to shaping our communities, including their vital role in advancing healthcare. Women have long been leaders in providing compassionate care, improving medical practices, and expanding access to health services for communities in need.

One trailblazer was Rebecca Lee Crumpler. In 1864, she became the first African American woman in the United States to earn a medical degree. At a time when opportunities for women, especially Black women, in medicine were extremely limited, Dr. Crumpler broke barriers and dedicated her work to caring for underserved communities.

Her legacy continues to inspire generations of women pursuing careers in healthcare and working to improve the well-being of others.

Durante el mes de la historia de la mujer, reconocemos las importantes contribuciones que las mujeres han hecho para fortalecer nuestras comunidades, incluyendo su papel fundamental en el avance de la atención médica. Durante generaciones, las mujeres han sido líderes en brindar atención compasiva, mejorar las prácticas médicas y ampliar el acceso a servicios de salud para las comunidades que más lo necesitan.

Our mission is to deliver health and hope for Chicago’s underserved communities.

In 2004, residents of the neighborhoods of Pilsen and Little Village on Chicago’s Southwest Side joined administrators from nearby Saint Anthony Hospital to address the critical lack of accessible health services in the area. They opened a community health center on South California Avenue called Esperanza, the Spanish word for “hope.”

Today, Esperanza provides bilingual, high quality primary care, behavioral health and wellness services to the community, regardless of immigration status, insurance status, or ability to pay. Esperanza’s four sites in Brighton Park, Little Village, and Marquette Park deliver care to over 20,000 patients each year from communities across the Southwest Side.

In May, we opened a fourth, and largest, site in Brighton Park to serve an additional 20,000 patients each year with high quality healthcare. Our commitment to offering care of the highest quality is made clear by our designation as a National Quality Leader by HRSA in both 2016 and 2017, which ranks Esperanza among the top 3% of health centers in the nation and the top two in Illinois for our quality of care.
